CEZ ID Cards Application
CAVITE ECONOMIC ZONE ID CARDS APPLICATION
Only the CEZ Locator's Authorized Representatives (CLAR) are allowed to send applications to cezssd@peza.gov.ph.
REQUIREMENTS:
1. Duly accomplished CEZ ID Application Form
2. Digital ID Photo of requested employee.
CEZ Vehicle Sticker Application
CAVITE ECONOMIC ZONE VEHICLE STICKER APPLICATION
CEZ Locators' Employees are required to fill-out the CEZ.8.F.005 - (CEZ Vehicle Sticker Application - CEZ Locator's Employee Form) and this should be submitted to the CEZ Locator's Authorized Representative (CLAR).

Only the CEZ Locator's Authorized Representatives (CLAR) and Service Providers are allowed to fill-out CEZ.8.F.017 (CEZ Vehicle Sticker Application - Group Form) and send applications to cezssd@peza.gov.ph.
REQUIREMENTS:
CEZ Locators’ Vehicle
1. Duly accomplished “CEZ.F.8.017 - CEZ Vehicle Sticker Application - Group Form”
2. Photo/scanned copy of vehicle O.R. / C.R. If necessary, attach document/s that support/s the ownership of the vehicle (e.g. Deed of Sale, Certificate of Sale, etc.)
3. Fee of P100.00 per sticker (motorcycles and four-wheeled vehicles), P1,570.00 per sticker (for trucks).
Employees' Vehicles
1. Duly accomplished “CEZ.8.F.005 CEZ Vehicle Sticker Application - CEZ Locator's Employee Form”
2. Photo/scanned copy of vehicle's valid LTO Official Receipt (O.R.) and Certificate of Registration (C.R.). If the vehicle is not registered under the applicant, attach photo/scanned copy of the following:
a. Duly Notarized Deed of Sale from Registered Owner / or
b. Certificate of Sale/Repossession or Authority to Use for partially paid 2nd hand vehicles from Vehicle Dealers / or
c. Marriage Certificate for Spouses / or
d. Birth Certificate for relationship basis with First-Degree relatives / or
e. Certificate of Cohabitation for Common Law Partners.
3. Photo/scanned copy of valid Zone Pass ID or valid Temporary Pass ID.
4. Photo/scanned copy of applicant's valid Driver's License or his/her Authorized Driver (Attach authorization letter stating
applicant's affinity with the authorized driver)
5. Fee of P100.00 per sticker.
Service Providers’ Vehicles
1. Duly accomplished “CEZ.F.8.017 - CEZ Vehicle Sticker Application - Group Form”
2. Photo/scanned copy of vehicle O.R. / C.R. If necessary, attach document/s that support/s the ownership of the vehicle (e.g. Deed of Sale, Certificate of Sale, etc.)
3. Proof of conduct of business inside CEZ which can be one of the following:
a. Certification from client CEZ Locator
b. Valid PEZA Registration / Accreditation
c. DENR Permit to Transport which states a Client CEZ Locator (for Hazardous Waste Haulers
4. Fee of P400.00 per sticker (motorcycles and four-wheeled vehicles), P1,570.00 per sticker (for trucks).
